(ICG) Commitment to the principles of Responsible Distribution® and the protection of Public Safety and Health, our firm asks the organizations who resell our products to perform a Self-Assessment to enhance their awareness of Chemical Distribution practices.

Please provide this form to the person in your organization who is responsible for your Product Stewardship Activities.

To apply for a new line of credit with AllChem submit this 3 page credit application.

Credit Application AIHC

We will also require your State Sales Tax Exemption Certificate and “for resale or manufacture”.  Please submit to Shari.Dixon@AllChem.com

Payment can be processed via ACH/Wire Payments.

Remit Information should be sent to ARRemit@AllChem.com.

Bulk Delivery Checklist

To ensure that our carriers are instructed properly, it is our policy to get a delivery checklist for every offloading terminal that we service.

This form will be sent directly to the carrier we contract with to complete the order.  Product cannot be delivered without this completed form, but after the initial form is submitted it remains on your customer account for future deliveries to the same destination.
